bullet Blank Birth Certificate Form जन्म प्रमाण पत्र का प्रपत्र
 
 Death Certificate
bullet Certificate from Crematorium for the Deseased मृतक के लिए शमसान द्वारा निर्गत किए गए प्रमाण पत्र
bullet Certificate from Doctor for the deseased मृतक के लिए डाक्टर द्वारा निर्गत किए गए प्रमाण पत्र
bullet Blank Certificate Form मृत्यु प्रमाण पत्र का प्रारुप
 
 Caste Certificate
bullet Residential Certificate आवासीय प्रमाण पत्र
bullet Photo copy of Khatiyan showing Caste चालु खतियान (जाति दर्शाते हुए) का फोटो कॉपी
bullet Any Revenue Record where caste of Applicant is mentioned आवेदक का जाति दर्शाते हुए कोई भी राजस्व रिकॉर्ड
bullet Proof of Relationship of applicant with the Khatiyan Owner खतियान धारक से आवेदक का रिस्ता का प्रमाण
bullet Affidavit by the Applicant stating the authenticity of the enclousers दिए गए अनुलग्नक हेतु आवेदक द्वारा शपथ पत्र
bullet In case Government Official copy of Service Book mentioning the Caste सरकारी कर्मचारियों के लिए सेवा पुस्तिका (जाति दर्शाते हुए) का फोटो कॉपी
 
 Income Certificate
bullet Affidavit by Guardian in case Applicant is less than 18 years. 18 वर्ष से कम आयु के आवेदक के लिए अभिभावक द्वारा सपथ पत्र
bullet For Government Employees photo copy of Pay Slip सरकारी कर्मचारियों के लिए पे-स्लिप का फोटो कॉपी
 
 Residential Certificate
bullet Photo Copy of Khatiyan as per Recent Survey data अद्दतन चालु खतियान का फोटो कॉपी
bullet Revenue Rent Receipt राजस्व लगान का रसीद
bullet Photo copy of Birth Certificate जन्म प्रमाण पत्र का नकल
bullet In case the residential certifcate is required for education purpose proof of education in the concerned place for atleast 10 years शिक्षा हेतु आवासीय प्रमाण पत्र के लिए कम से कम एक स्थान पर दस साल का शिक्षा प्राप्ति का प्रमाण
bullet Photo copy of Ration Card or Voter Id Card राशन कार्ड अथवा वोटर पहचान पत्र का फोटो कॉपी
bullet Affidavit by Guardian in case the Applicant is below 18 years 18 वर्ष से कम आयु के आवेदक के लिए अभिभावक द्वारा शपथ पत्र
